Job Description:
We are seeking a Home Health Registered Nurse to provide compassionate, patient - centered care in home settings. This role involves delivering high - quality nursing services and collaborating with healthcare teams.

Key Responsibilities
• Provide nursing care in patients' homes.
• Use HCHB/OASIS for documentation.
• Develop and implement care plans with healthcare teams.
• Conduct assessments and monitor patient health.
• Educate patients and families on care procedures.
• Travel within Oakland, Wayne, and Macomb counties; mileage reimbursed.

Requirements:
• Active RN License and BLS Certification.
• 2+ years of home health RN experience.
• Proficient in HCHB/OASIS.
• Flexible hours; early mornings or late evenings possible.
• Reside within 30 minutes of the territory.

Additional Information:
• 13 - week contract, 40 hours/week, no on - call or overtime.
• Weekly in - office presence required.
